The U.S. Copyright Office describes substantially similar requirements for effective DMCA notices under Section 512.

3. Removal of Allegedly Infringing Material
Upon receiving a sufficiently complete copyright infringement notice, we may review the claim and take appropriate action, which may include removing or disabling access to allegedly infringing material.
We do not guarantee that every claim will result in removal.
4. Counter-Notification
If material was removed or access was disabled as a result of a copyright complaint and you believe the removal was made in error or that you have authorization to use the material, you may submit a counter-notification where applicable under the DMCA.
A valid counter-notification generally requires information specified under applicable law.
5. Repeat Infringers
Where appropriate and at our discretion, HealKoreaMag may restrict or terminate access for individuals or accounts that repeatedly engage in copyright infringement.
6. Our Own Copyrighted Material
Unless otherwise stated, original articles, photographs, graphics, logos, and other original materials created for HealKoreaMag are protected by applicable copyright law.

8. DMCA Designated Agent
If HealKoreaMag elects to seek DMCA safe-harbor protection under 17 U.S.C. § 512(c), the designated agent information should be registered with the U.S. Copyright Office and kept current.
The U.S. Copyright Office requires covered service providers seeking the applicable safe harbor to designate an agent and make the required agent information publicly available.
